Elevate Support Care NSW Pty Ltd is seeking a compassionate and motivated Community Worker to support individuals, families, and community groups by providing guidance, advocacy, and access to essential services. The successful candidate will work closely with community members to assess needs, promote wellbeing, facilitate programs, and build strong relationships with local stakeholders.
Tasks & Responsibilities- Provide support and guidance to individuals and families within the community.
- Conduct needs assessments and develop appropriate support plans.
- Connect clients with relevant services, programs, and resources.
- Facilitate community engagement activities, workshops, and outreach programs.
- Maintain accurate records, reports, and case notes.
- Advocate on behalf of clients to ensure access to appropriate services.
- Collaborate with community organizations, government agencies, and service providers.
- Promote inclusion, diversity, and community participation.
- Certificate, Diploma, or Degree in Community Services, Social Work, Human Services, or a related field.
- Previous experience in community services, support work, or a similar role is preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively with people from diverse backgrounds.
- Excellent organizational and problem-solving abilities.
- Knowledge of local community resources and support services.
- Ability to maintain confidentiality and professional boundaries.
